This form is used to assess suitability for EPIC’s Mental Health Service and determine the appropriate service stream. It is intended for use when the referrer (or individual) is ready to proceed and has sufficient information to support intake suitability screening.
Completion of this form does not guarantee acceptance of a referral. All referrals are subject to screening and service scope confirmation.

Important notes for referrers
- EPIC’s Mental Health Service provides counselling and mental health support for individuals experiencing emotional distress, life stressors or periods of transition, where short-term, goal-focused counselling is appropriate.
- EPIC’s Mental Health Service is not a crisis or acute mental health service. Referrals where immediate safety concerns are present may not be accepted and may be redirected to appropriate crisis or specialist services.
- We do not provide diagnosis, assessment for diagnosis, or intensive clinical treatment.
- Referrals are subject to intake and suitability screening.
Delivery of services
EPIC’s Mental Health Service is delivered primarily via telehealth.
In-person options may be available where appropriate and subject to suitability, availability and approval.
Important notes regarding in-person delivery
- Telehealth is the preferred and default delivery model across all service streams.
- In-person sessions at EPIC’s Nundah office or at a referrer organisation may be considered where clinically and operationally appropriate.
- In-person sessions delivered outside EPIC’s Nundah office may be considered subject to specific criteria.
- Additional charges may apply for outreach sessions, including distance-based or time-based costs, depending on location and funding stream.
- All in-person arrangements must be pre-approved prior to booking.
When EPIC’s Mental Health Service is a good fit
EPIC’s Mental Health Service provides short-term counselling and mental health support aligned to employment participation and work readiness.
This service supports IEA participants to stabilise wellbeing, build coping strategies, and address barriers impacting engagement with work or employment pathways.
Important note for IEA referrers
EPIC’s Mental Health Service provides short-term, telehealth-first mental health support aligned to employment goals. The service is non-crisis and does not provide diagnosis, acute intervention, or long-term therapy. Referral acceptance is subject to intake and suitability screening.
EPIC’s Mental Health Service provides short-term, capacity-building mental health support that complements other NDIS or clinical services.
The service focuses on functional wellbeing, participation, routines, coping strategies and employment-related supports — not therapeutic treatment or diagnosis.
Important note for NDIS referrers
This service is short-term and non-crisis. EPIC’s Mental Health Service does not provide acute mental health care, diagnosis, intensive therapy, or multidisciplinary clinical intervention. All referrals are subject to intake and suitability screening.
When EPIC’s Mental Health Service is a good fit
EPIC’s Mental Health Service provides short-term counselling and mental health support for individuals who would benefit from a supportive, non-clinical service focused on wellbeing, participation, and functional outcomes.
This service complements — and does not replace — clinical treatment or therapy.

EPIC’s Mental Health Service provides Counselling & Mental Health Support for individuals who are experiencing emotional distress, life stressors, or periods of transition, and who would benefit from professional counselling support.
The service offers a supportive and collaborative space to talk through concerns, make sense of what’s happening, and work toward greater stability, clarity and wellbeing.
This service provides short-term, non-crisis counselling and mental health support.
It is not suitable for urgent mental health care, diagnosis, or intensive long-term therapy.
